By using the services of Bentley Critical Incident Response, I acknowledge and agree to the following:

Legal Representation

1. I understand that it is essential that I obtain legal counsel before being interviewed about what occurred during my critical incident.
2. I understand that Jim Bentley does not represent me unless I have personally spoken with him either in person or by telephone. Jim Bentley is not my attorney unless he has expressly informed me that he is acting as my attorney and has met with me before I am interviewed regarding my critical incident.
3. If I have not spoken with Jim Bentley before being interviewed about what occurred during my critical incident, I understand that I must contact an attorney who can appear and represent me before any interview takes place.
4. I recognize the importance of having legal representation before being interviewed. I will ensure that I have an attorney representing me before participating in any interview concerning my critical incident.
